The position of Registered Nurse is at the forefront of providing homecare in a national organization, delivering RN nursing care to patients who are rehabilitating, managing chronic conditions or are in the end stage of life. The RN position involves significant nursing responsibility for patient care and helps make a positive impact on patients served and the organization.
Provides care and education to patients and their caregivers
Participates in the coordination of services with other disciplines and community resources as needed
Evaluates and revises care plans/assessments based on changes in patients and/or their environments
Participates in regular case conferences and in-service or special case orientations as needed and participates in on-call rotation.
Graduate of an accredited school of nursing
Currently licensed to practice in Indiana
Minimum 1 year of RN experience.
Previous home health experience preferred
