Performs visits as prescribed in the physician’s plan of treatment and in compliance with state and federal guidelines. Makes skilled observations regarding patient’s physical and mental condition.
Provides patient care and education, monitors patient status, responds to treatment/care, reports changes in patient’s condition, completes and submits appropriate and timely written documentation.
Participates in coordination of services with other disciplines and community resources.
Participates in case conferences, attends in-services, and/or special case orientation.
Demonstrates sensitivity to patient and/or family needs, customs or feelings.
Completes and submits all required documentation in a timely manner.

Graduated from an accredited school of practical nursing.
Currently licensed to practice in the state serviced. Proof of eligibility to pass medications required. One year of nursing experience required.
Current CPR certification (all locations).
One year home care experience required.
