Job Summary / PurposeThe Community Health Worker (CHW) is at the forefront of improving patient’s health. Developing and expanding home care programs which serve patients appropriately, and efficiently is a priority for the organization and the CHWs play a vital role. The CHW works in the field in a variety of settings, including but not limited to patient homes, medical day centers, and homeless shelters. The CHW is a member of the care management multidisciplinary outreach team. The CHW will assist with care plan implementation, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of at risk patients.



  • Certified Medical Assistant or LPN preferred; other backgrounds will be considered.

  • Experience assisting populations with health, social and financial barriers in the inpatient, outpatient or community setting.

  • BLS or completion within one month of hire.

  • Completion of Competency course for CHW within three months

  • Ability to effectively provide support to socially and medically complex patients

  • Exceptional organizational and interpersonal skills with attention to detail

  • Strong oral/written communication skills

  • Ability to work collaboratively in a team and manage multiple priorities, utilize effective time management skills, exercise sound administrative and clinical judgment

  • Demonstrated ability to work well with people of various ages, backgrounds, ethnicities, and life experiences

  • Ability to travel to multiple office locations, valid driver’s license and automobile that is insured
