Experience Required.Benefits: (90 day waiting period required):Health, Dental and Vision InsurancePaid time offMonthly Mileage StipendAbility to work from homeTypical Schedule: Monday through Friday (Requires being available for on call emergencies/supports-including weekends for assigned homes)Position Summary:i Program Coordinator is responsible for the oversight and coordination of services provided to P and P Home Servicesi (PPHS) clients.i The Program Coordinator supervises Direct Care Professionals (DSP) and provides training, mentoring, coaching and positive supervision to staff on an ongoing basis.i Specifically, the Program Coordinator is responsible for ensuring DSPs are fully and appropriately trained, documentation is current and accurate, services provided are in full compliance with all state and federal laws, the consumeris PCISP, BSP and other applicable plans.iThe Program Coordinator will have a specific caseload, specialized assignments and are assigned to the North, East, South, West and Central areas within the state of Indiana.iResponsibilities/Duties: The following job responsibilities/duties are general in nature and should be considered guidelines, and not a definitive list.i Program Coordinators are asked to work together in a team environment and, as a result, may be asked to assume additional responsibilities deemed necessary that are not listed herein.Direct, manage and administer the coordination and service delivery components of programs that service consumers in residential settingsSupervise the delivery of service and ensure individuals and consumers are receiving the highest quality careRoutinely observe service delivery on-site and monitor for demonstration of knowledge of consumer health needs, behavior management techniques, and emergency procedures.iServe as a liaison between the community and agencies in the service delivery system, family/guardians, and the agencyCommunicate/collaborate with other providers, families, and consumers.Report, identify and resolve conflicts between staff members, clients, and client family members and/or guardiansAbility to be on call for emergencies for the homes assigned on their caseloadComplete IRsTrain, evaluate, and monitor assigned DSPisComplete Performance Evaluations to ensure employees are productive, accountable, and successful in their positionsEnsure tasks set out by the program Coordinatoris Expectations and Task Form are completed in a timely and professional mannerConduct and/or monitor training for all staff in the least restrictive techniques, behavior management, active treatment, client rights, prevention of abuse/neglect, documentation/data collection, emergency procedures, and other areas as neededWork with Program Director and Program ManagerConduct regular in-home visitsComplete quarterly reports and other reportsReview and approve staff timeComplete schedulesMeet with PM regularly (virtually and in person)Participate in care coordinationAttend consumeris quarterly and annual meetings, and other meetings, as necessaryMake recommendations regarding promotions, compensation, bonuses, staff recruitment, retention, and disciplinary actionsParticipate in transitionsAttend trainings and workshops offered by the State of Indiana and other resources pertaining to the jobComplete Risks Assessments/PlansCore Competencies (Knowledge, Skills, and Abilities)Must show a genuine interest in and empathy for persons with intellectual and physical special needsStrong verbal and written communication skills, including the ability to adapt communication style and content depending on audience (e.g., client, guardian, case manager)Strong leadership skills to oversee diverse staffAbility to schedule and manage tasks effe
